The following documents are being submitted for approval of use of Lions name and logo as it applies to the formation of a Lions Club Foundation (Oregon Springfield Lions Foundation also referred to as OSLF). The creation of this Foundation started back in 2018 but has been delayed by edification on forming a Foundation and now COVID-19. Per required LCI submission reporting the following documentation(s) are being forwarded.

Copy of IRS Form 1120 as requested by IRS and outlined in the EIN determination letter.
Letter of explanation which accompanied Form 1120 relating to the effects COVID-19 had regarding fundraisers nonexistence hence $0.00 balance for the fiscal year. District 36M-R Treasurer, certified CPA, did not believe that our corporate type indicated that this filing was required but agreed with us that filing should be made. Awaiting a response from the IRS as to whether this form's filing is required in the future.
